Eye Associates of North Florida is excited to offer attendees of the 2025 Tallahassee Tennis Challenger an exclusive chance to win $1,000 off LASIK and a free pair of sunglasses (up to $250). This event prize is our way of helping you achieve clear vision while protecting your eyes in style. 

1. ELIGIBILITY

The 2025 Tallahassee Tennis Challenger LASIK Prize Winner (the “Prize”) is open to: 

• Legal residents of the United States who are at least 21 years old at the time of entry. 

• Individuals who meet the medical eligibility criteria for LASIK surgery, as determined by a licensed LASIK specialist during a mandatory free consultation. 

• Attendees of the Tallahassee Tennis Challenger who complete the entry form in person or online. 

Employees, agents, and representatives of Eye Associates of North Florida, as well as their immediate family members (spouses, parents, siblings, children, and household members), are not eligible to participate. 

2. HOW TO ENTER

Participants must submit a completed entry form either: 

• In person at the Eye Associates of North Florida booth at the Tallahassee Tennis Challenger. 

• Online by scanning the QR code available at the event and filling out the form. 

Deadline for Entries: April 20, 2025, at 11:59 PM EST. Late or incomplete entries will not be considered. 

3. WINNER SELECTION PROCESS

• Random Drawing: The winner will be selected in a random drawing from all eligible entries received by the deadline. 

• Winner Notification: The selected winner will be notified via phone and/or email within 7 days after the giveaway closes. 

• Claiming the Prize: The winner must confirm acceptance and schedule their LASIK consultation within 30 days of being notified. 

• Medical Eligibility: The winner must complete a free LASIK consultation to confirm they are a suitable candidate for the procedure. If the winner does not qualify for LASIK due to medical reasons, no alternative prize will be offered. 

4. PRIZE DETAILS & RESTRICTIONS

• Prize: One (1) winner will receive $1,000 off LASIK surgery and a free pair of sunglasses (up to $250 value). 

• No Cash Value: The prize is non-transferable, non-exchangeable, and cannot be redeemed for cash. 

• Location & Timeline: The LASIK procedure must be scheduled and completed at Eye Associates of North Florida at 2020 Fleischmann Rd, Tallahassee, FL 32308 within 6 months of being selected as the winner. 

• Additional Costs: Any costs exceeding the $1,000 LASIK discount or $250 sunglasses value will be the responsibility of the winner. Financing options may be available for additional treatment needs. 

• Taxes: All federal, state, and local taxes, if applicable, are the responsibility of the winner. 

5. GENERAL TERMS

• No Purchase Necessary: Participation in this event prize is free and does not require a purchase. 

• Privacy & Consent: Submission of your completed registration form constitutes your express consent and permission for the sponsor to use your name, photographs, videos and written responses (in whole or in part) for promotional and advertising purposes to promote Eye Associates of North Florida, its business, and the Prize, in any media and in any manner whatsoever, as well as it’s vendors and affiliates without limitation or restriction, and such consent is deemed given by your entry and participation in the Prize. Submission of your completed entry shall be deemed your acceptance of these Rules and Regulations (as may be amended) and your voluntary transfer to the Eye Associates of North Florida of all right, title, and interest, including copyright, of your photographs, videos and written responses. All information and materials submitted become the sole and exclusive property of Eye Associates of North Florida. 

• Disqualification Rights: Eye Associates of North Florida reserves the right to disqualify any entry that is incomplete, fraudulent, or does not meet eligibility requirements. 

• Liability Release: By participating, entrants and the winner agree to release the Eye Associates of North Florida, affiliated companies, and the directors, officers and employees of any of them from all risk, loss, injury, damage or harm that may arise from participating in the giveaway or the LASIK procedure. 

• Modification Rights: Eye Associates of North Florida reserves the right to interpret these rules and, if necessary, to amend the rules at its sole discretion and without notice to individual participants. Any amendments to the rules or changes in prizes will be published online at www.eyesnf.com. Rule interpretations and all judging decisions are final.
