That’s why we’re offering a deserving firefighter (or EMTs and paramedics) the life-changing gift of free LASIK surgery. This giveaway is our way of giving back to those who serve our communities every day. 

If you know someone who deserves this opportunity, nominate them by submitting an short essay explaining why they should win. Eye Associates of North Florida will carefully review each nomination and select the winner. 

Of course, there are a few important rules and guidelines to keep everything fair and transparent. We’ve outlined all the details below—so keep reading to learn more about eligibility, submission deadlines, and what to expect if you’re chosen! 

FREE LASIK FOR A FIREFIGHTER GIVEAWAY 

1. ELIGIBILITY

The Free LASIK for a Firefighter Giveaway (the “Giveaway”) is open to: 

• Active career, volunteer firefighters, Emergency Medical Technicians (EMTs), or paramedics (the “firefighter”) in the United States.  

• Individuals who are at least 21 years old at the time of entry. 

• Firefighters who meet the medical eligibility criteria for LASIK surgery, as determined by a licensed LASIK specialist during a mandatory free consultation. 

Verification Requirements:

To enter, nominees must provide one (1) of the following to verify their firefighter status:

– Official Fire Department ID Badge OR
– Employment Verification Letter from their Fire Chief or Department Supervisor OR
– Firefighter Certification Number (if applicable) OR
– Roster Verification (for volunteer firefighters) 

Failure to provide proof of valid status will result in disqualification. 

Employees, and agents of Eye Associates of North Florida, and members of their immediate family (including spouses, parents, siblings, children and their respective spouses) and persons living in the same household with any such individuals are ineligible. 

2. HOW TO ENTER

To nominate a firefighter, participants must submit: 

1. A written essay explaining why the nominee deserves free LASIK surgery. 

1. Contact information for both the nominee and nominator (if different). 

Deadline for Entries: April 15, 2025, at 11:59 PM EST. Late or incomplete entries will not be considered. 

3. WINNER SELECTION PROCESS

• Judging Panel: Eye Associates of North Florida staff will select the winners in the contest from all eligible and completed entries. Finalists may be subject to an interview process before the giveaway is awarded. 

• The selected winner must complete a free LASIK consultation to confirm eligibility for the procedure. 

4. WINNER NOTIFICATION

• The winner will be notified by phone and/or email within 14 days after the deadline. 

• If the winner does not respond within 5 business days or does not qualify for LASIK due to medical reasons, an alternate winner may be selected. 

• The winner must schedule and complete their LASIK consultation within 30 days of being notified. 

5. MEDICAL ELIGIBILITY REQUIREMENT

• The winner must be medically eligible for LASIK surgery as determined by a licensed LASIK specialist during the free consultation. 

• Standard LASIK surgery policies and medical guidelines apply to all LASIK procedures performed, and the winner must adhere to any instructions given by the medical provider. 

• If the winner does not qualify for LASIK due to medical reasons, no alternative prize will be offered. 

• The winner must sign a waiver acknowledging the risks and benefits of LASIK before undergoing the procedure. 

6. PRIZE DETAILS & RESTRICTIONS

• The prize consists of one (1) free LASIK procedure for the selected firefighter. 

• The prize has no cash value and cannot be transferred, substituted, or redeemed for cash. 

• All winners are responsible for all applicable taxes. 

• The LASIK procedure must be scheduled and completed at Eye Associates of North Florida at 2020 Fleischmann Rd, Tallahassee, FL 32308 within 6 months of being selected as the winner. 

• Standard LASIK surgery policies, including any pre-operative and post-operative care, will apply. 

• Financing options may be available for additional treatments or if the winner chooses a higher-tier procedure. The winner will be informed of all applicable options during the consultation.
  
  
   

7. GENERAL TERMS

• No Purchase Necessary – This giveaway is 100% free to enter. 

• Privacy & Consent – Submission of your completed registration form constitutes your express consent and permission for the sponsor to use your name, photographs, videos and written responses (in whole or in part) for promotional and advertising purposes to promote the Sponsor, its business, and the Giveaway, in any media and in any manner whatsoever, as well as it’s vendors and affiliates without limitation or restriction, and such consent is deemed given by your entry and participation in the Giveaway. Submission of your completed entry shall be deemed your acceptance of these Rules and Regulations (as may be amended) and your voluntary transfer to the Sponsor of all right, title, and interest, including copyright, of your photographs, videos and written responses. All information and materials submitted become the sole and exclusive property of the Sponsor. Once pictures and/or videos are submitted as part of the competition they become the sole property of the Sponsor and may not be used in another LASIK giveaways. 

• Disqualification Rights – The Sponsor reserves the right to disqualify any entry that is incomplete, fraudulent, or does not meet eligibility requirements. 

• Liability Release – By participating, entrants and the winner agree to release the Sponsor, affiliated companies, and the directors, officers and employees of any of them from all risk, loss, injury, damage or harm that may arise from participating in the giveaway or the LASIK procedure. 

• Modification Rights – The Sponsor reserves the right to interpret these rules and, if necessary, to amend the rules at its sole discretion and without notice to individual participants. Any amendments to the rules or changes in prizes will be published online at www.eyenf.com. Rule interpretations and all judging decisions are final.
