By participating in this offer, you agree to the following terms and conditions: 

1. ELIGIBILITY

• Participants must qualify for LASIK surgery based on a consultation and medical evaluation. 

• Qualification is determined solely by the LASIK provider based on medical criteria. 

• Participants must be at least 21 years old and legal residents of the United States. 

• Employees of Eye Associates of North Florida, its affiliates, and their immediate family members are not eligible for this promotion. 

2. LOCATION REQUIREMENT

• The LASIK procedure must be performed at Eye Associates of North Florida to be eligible for the $1,000 credit and free sunglasses (up to $250 retail value). 

• Offer is only valid for procedures scheduled and completed within the promotional period. 

3. CONTENT SUBMISSION REQUIREMENT

• To receive the offer, participants must submit a photo or video explaining why they chose to undergo LASIK at Eye Associates of North Florida. 

• By submitting content, participants grant permission for their name, likeness, and submission to be used in promotional materials, including but not limited to social media, website, and advertisements. 

• Submission of content is not required for eligibility but is necessary to redeem the full offer. 

4. OFFER DETAILS

• The $1,000 credit is applied toward the total cost of the LASIK procedure and has no cash value. 

• The credit is non-transferable and cannot be combined with other discounts or promotions. 

• Free sunglasses (up to a $250 retail value) will be provided only after the procedure is completed and cannot be exchanged for cash or applied toward the surgery cost. 

• If a participant is deemed medically ineligible for LASIK, no alternative compensation will be provided. 

5. LIMITED AVAILABILITY & DETAILS

• This promotion is subject to change or discontinuation at any time without prior notice. 

• The offer is valid only for new patients who schedule their consultation and procedure within the promotional period. 

• To claim the offer, participants must schedule their consultation by [insert date] and complete their LASIK procedure by [insert date]. 

6. ADDITIONAL TERMS & CONDITIONS

• Standard LASIK surgery policies, including pre-operative and post-operative care, financing options, and medical guidelines, apply. 

• Participants must comply with all pre-operative and post-operative care instructions provided by their LASIK specialist. 

• Eye Associates of North Florida reserves the right to modify these terms and conditions at any time. 

• By participating in this offer, entrants agree to release Eye Associates of North Florida and its affiliates from any liability related to participation in the promotion or the LASIK procedure.
